Discrimination Report

Before you report discrimination using this form, you must answer these questions:

Have you already filed a lawsuit in State court about the discrimination you are about to report to us?
This question is required. Select Yes if you have filed a lawsuit, or No if you have not.
Do you have a claim or complaint about the discrimination you are about to report to us that is currently pending with another State or local agency?

Note: for employment discrimination, this does not include any complaint you may have filed with the EEOC.
